  • In the state of Illinois, referrals are no longer needed prior to being assessed and evaluated by a Physical Therapist. However, we will reach out to your Primary Care Provider and or Related Specialist after your initial evaluation to ensure we are an active part of your health care team.

  • In network with Blue Cross Blue Shield PPO and Medicare

  • Private PT sessions are available by appointment only- please check our online scheduler to book a PT session with one of our therapists.

    All Pilates group classes can be booked through the app ( Momence) or our website. To book a private Pilates session please email: info@reformphysicaltherapy.com

  • Physical Therapy:

    We encourage our patients to cancel no less than 24 hours prior to a scheduled visit due to an ongoing waitlist and small private clinic setting. If the cancellation occurs within < 24 hrs the patient will be charged a $100 cancellation fee.

    Pilates:

    We observe a 16hr cancellation policy for group classes + private sessions. Please cancel prior to 16 hrs or 1 class/private session credit will be deducted from your membership. If you are a no show, you will be charged an additional $12

  • PT: Please bring any diagnostic reports (ie: MRI, xray, written reports) in relation to injury or pain. If you are active or have a sports related injury, please bring the shoes that you are utilizing for sport and any orthotics/bracing you may be wearing for physical activity so your physical therapist may assess.

    Pilates: anything you would wear to a group fitness class is acceptable. Please no zippers or buttons. We do require grip socks ( if you chose to wear toe nail polish!) You can purchase them in studio :)

  • As a graduate of Northwestern University Feinberg School of Medicine, Meg has the background and experience to identify musculoskeletal deficits and treat you, the individual; offering 1:1 private PT sessions.

    Meg is also pursuing her Pilates certification through The Lab; integrating Pilates into your plan of care.

  • We recommend scheduling a private Physical Therapy session or private Pilates session to assess current abilities/needs.

  • Physical Therapy

    • Upper body injuries: Tank top/Sports Bra
    • Lower body injuries: Shorts

    ** Layering up is totally fine!
